Step 1
Complete Intake Interest Form
Tell us a little about your child and family by completing our online intake form.
This helps us learn more about your concerns, answer your initial questions, and begin the process of finding the right services for your child.
Step 2
Request a Referral
Please contact your child's primary care physician and request a referral for speech and/or occupational therapy.

Email Your Insurance Card
Please email a copy of the front and back of your child's insurance card to: info@talkingtidestherapy.com
If your child has previously completed an evaluation, received a diagnosis, or has other relevant records you'd like to share, you're welcome to include those as well. These documents can be helpful when verifying benefits and planning your child's care.
